MLS Clinical and Certificate Application 

Applicants for the BS-MLS clinical year and the certificate track apply during the fall semester for the following academic year. The clinical year begins once a year, only in the fall. Students should allow adequate time to complete the application, which includes procuring three references (at least two of which should be from science faculty) and completing a health form.

Transcripts

For UC students, we will access your official record of UC courses. Please see also the directions below for sending fall grades for any courses taken outside of UC.**

Your UC-student record includes any courses taken at other colleges/universities IF you previously sent them to UC as official transcripts. If you are unsure whether UC received transcripts of external courses, contact your academic advisor who can show you how to find this information. If you discover that you need to send transcripts to UC:

All Non-US University Transcripts require an accompanying course-by-course evaluation by a member of the National Association of Credit Evaluators (naces.org), such as World Education Services (wes.org).  The evaluation must include the number of credit hours taken and a cumulative GPA. Have the evaluator send both the original official transcripts and the course-by-course evaluation to:

** Grades for the Fall Semester in Which You Are Applying – As soon as grades post for the Fall semester of application, non-UC students must send an unofficial copy of their grades including current cumulative/overall GPA statistics. This applies to UC students ONLY if they took courses at another institution during the Fall semester of application since the program can access students’ UC fall grades.

** Email Fall grade reports to: MLS-Campus-Based@uc.edu​ Include a Subject line with your first and last name, with your UC M# in the body of your email. 

We accept unofficial reports for external (non-UC) Fall semester grades to expedite course evaluation and to save applicants the cost of sending additional transcripts. If students are accepted into clinicals, we require complete official transcripts ​of all non-UC courses taken prior to starting the clinical year, to verify any grades sent unofficially. 

Follow the Steps below to Complete the MLS Clinical and Certificate Application

The application is an online form, broken into a series of four ​sections below to allow easier completion. Each ​section must be completed before proceeding to the next: you will not be able to save an incomplete ​section to finish it later, although you can complete each ​section separately, at different times if needed. Each section takes about 5 minutes to complete - this does not factor in the time needed to complete any Word file you will submit within the form. 

Sections 1, 2 and 4 have Word files (docx) ​which are required for you to submit within their respective sections. Complete these forms prior to starting the form for that section of the application. You may complete the Word files ahead of time and then submit them later while completing the application. 

  1. Click on the Word DOCX filename to download the file to your computer. Open it and use Save As to re-name it and save it to your computer or cloud. 
  2. Name the file using this format: Last Name_First Name#  where # is the section number. For example, student with first name Ima and last name Labcat saving the Word file for section 2 would name their file as: Labcat_Ima2 
  3. Complete the downloaded Word file and save your updates. Then be prepared to access this file when you are ready to complete that ​section of the application. Once inside the form for that section, you will be prompted to upload your completed Word file. 
  4. After submitting a form, you will receive a message either confirming it was successfully submitted or alerting you that there was an error and it wasn't. 
  • When you are ready to begin the application, start with Section 1 below.
  • After submitting a form, please watch for the message at the top of the page (you may need to scroll up) letting you know if the form was submitted or not. If not, you should try submitting it again before contacting the Help Desk: (513) 556-HELP (4357) -OR- (866) 397-3382. Select option 2 in the menu. 
  • You will need to return to this page to access the forms in Sections 2, 3 and 4. The form is Section 4 is not submitted online but is emailed to the account listed or faxed to the number listed.

 

Section 1 - Demographic Information, Education & Experience

Before completing Section 1, download and complete the word document below following the numbered instructions at the start of the application. Click on the filename to download and save this Word file: MLS education-experience (DOCX)  

After you have completed the Word file and are ready to complete Section 1 of the application, click on the link Demographic Information Form below to access the form for section 1. Once inside, you will be submit your Word file in the Education &  Experience upload section at the bottom of the form.

Section 2 - Essential Functions & Questionnaire

Before completing Section 2, download and complete the word document below following the numbered instructions at the start of the application. Click on the filename to download and save this Word file: MLS Clinical Application Questions (DOCX)

After you have completed the Word file and are ready to complete Section 2 of the application, click on the link Essential Functions & Questionnaire Form below to access the form for section 2. Once inside, you will be submit your Word file in the Education &  Experience upload section at the bottom of the form.

Section 3 - Reference Agreement and Reference Evaluations

If you are the applicant: please complete the Reference Agreement form below to allow permission for your references to submit evaluation forms. After completion, send the URL to this application page to your references and instruct them to go to Section 3 where it reads,"If you are providing a reference for an applicant, please complete the Reference Evaluation Form below:"

Section 4 - Medical Physician Form 

Once you have completed steps 1 - 4, you can move on to the final step.

In order to apply for the program, we will need a Statement of General Health; form to be completed and signed by your physician.
