How to Apply

Certificate students apply by completing the MLS Clinical Application.

The application packet is currently unavailable while it is edited for the upcoming application cycle.​​

For the 2019-2020 year, the application will be located online in October for a deadline in December.  Specific deadlines will be listed once we post the application. We are moving to an online submission so please check back to see when the application becomes available. 

In the meantime, there are things you can start working on to complete your application:

  • Read the Frequently Asked Questions (FAQs) PDF below which answers many questions about the application process. 
  • ​You will need three references (at least two of which should be from science faculty) to complete a reference form for you. You can start by identifying people to ask to serve as your reference. Please read the FAQ section on references more about this.  ​If your potential reference agrees, let them know that you will NOT need a letter (which take some time to write and they are accustomed to sending for references) but they will use a form which will be available online with the application. The form requires minimal time to complete, unlike a letter which is not required.
  • If you are submitting international transcripts, those need a course-by-course credit evaluation (see Admission Requirements) so should be submitted by December 1 to allow for transcript evaluation.​Complete a health form which requires a signature from a healthcare provider. This form will also be available with the application. 
  • Update your resume or create a simple one.
    • List all of the higher education (colleges and universities) you have attended, along with dates (Month/Year) and major. Include your current institution, which might be UC.
    • List any jobs - paid or volunteer - that you wish to highlight with dates (Month/Year). 
    • Write down brief statements about why you are interested in the MLS profession and your educational and career plans. List anything you think we should know about accomplishments or challenges. Having some brief sentences in one place will help you respond to some questions that will be in the application when it becomes available.
