Certificate in Dietetics
How to Apply
Please follow the following steps to apply to the program:
- The application form must be typed or neatly printed in ink. Information included on the form must be accurate and complete. It is not necessary for applicants to apply to the University of Cincinnati separately.
- Applicants to the DPD Certificate must send official copies of all transcripts and a résumé with the application form. If applicants have had a transfer credit evaluation completed by the University of Cincinnati prior to applying to the DPD Certificate, a copy of the credit evaluation report can be submitted instead of official transcripts.
- A completed copy of the UC Admission Confirmation Form must also be submitted. The fee for application indicated on this form is waived for DPD Certificate applicants.
- The completed application packets for the DPD Certificate should be sent to:
