Clinical Education Requirements
Immunizations and Physical Exam
Students in the Doctor of Physical Therapy program are required to have the following vaccinations and an annual physical exam.
Required Documents:
- Health Professions Programs Standard Immunization Form. You must have your healthcare provider complete the mandatory Standardized Immunization Form. Immunization notes:
- Review the following Immunization Requirements. These requirements are also listed within the Health Professions Programs Standard Immunization Form.
- Hepatitis B - proof of shot series AND positive titer will be required. We recommend that you start this early in case you get a negative titer as you will need to re-complete the series as outlined in the immunization form.
- Measles, Mumps, Rubella (MMR) – student will choose one of the following options:
- proof of 2 doses of MMR vaccine OR
- two (2) doses of Measles, two (2) doses of Mumps and (2) doses of Rubella OR
- serologic proof of immunity for Measles, Mumps and/or Rubella via positive titer
- Meningococcal – if the student will be younger than 22 years of age at the start of your first semester with UC, the student will need to show proof of one dose given on or after 16th birthday
- Tuberculosis - proof of negative two-step (2) TB Skin Tests (TSTs) OR (1) IGRA blood test done within the past 3 months. Additional testing will be required throughout enrollment in the program.
- Tetanus, Diptheria, Accellular Pertussis (Tdap) – proof of 1 dose within the past 10 years
- Varicella – proof of 2 doses of vaccine OR positive titer
- COVID-19 vaccination – proof of primary series of two monovalent doses OR one bivalent dose
- Influenza (flu) – students will be required to complete this annually by October 31; more information to come from Director of Clinical Education
How to Submit:
- The university utilizes the Bearcat Health App to collect the immunization form. For additional information: Instructions for uploading Immunization Form & troubleshooting assistance
· You will also be required to submit all documents to Exxat for the Director of Clinical Education. Additional information on Exxat to come from Director of Clinical Education.
- Hold on to your original Standard Immunization Form. If updates need to be added (titer, flu shot, etc), include the updates on the original form with a new date and signature and upload it a second time.
- No additional documents can be uploaded with the Student Immunization Form to the Bearcat Health App (including a vaccination printout from your healthcare provider).
Physical Examination Form Notes:
- Please hold on to the hard copy of your Physical Exam Form once it is completed. The Physical Exam Form should NOT be uploaded to the Bearcat App. This will instead be submitted to Exxat. You will receive additional information from the Director of Clinical Education on accessing this site.
- Please make sure this form is filled out completely by your physician. Any missing entries will be required to be fixed before approval.
